It is important that you inform your GP about the medicines you take to ensure they have an accurate medication history.
Sharing this information with your GP means that the healthcare professional involved with your care would have all the necessary information to make informed clinical decisions about your care.
Additionally in the rare case of side effects we would be able to contact your GP and make them aware if needed.
You can opt out sharing your GP details by sending via “your messages” a recent full-length image of yourself together with a form of photo ID such as passport or driving licence on this email.
This is in order for us to verify that the BMI information we have for you is correct, and protect anyone who is not suitable for treatment.
